The change project will be based on six SMART goals. The goals will serve as a guide to ensure the project run smoothly and completed within a specified timeframe. In addition, each goal will be specific, measurable, achievable, relevant and time bound. The first SMART goal is to reduce fall in surgical patients in the medical surgical unit. In this case, a fall risk assessment will be conducted using validated tools such as Hendrich 11 model for new admitted patients. This would help surgical nurses to identify patients at risk factor for falls including history of fall, altered mobility, elimination or depression. Cuttler et al., (2017) also shows surgical patients are vulnerable to falls due to altered mobility, medical adverse effects, altered mental status, history of falls and frequent toileting. This goal will be achieved through huddles during each shift in the March 31, 2021.
